Reza Esfahani, DO, CMD, MSPH
Dr. Esfahani’s career in medicine began when he worked as a respiratory therapist at St. Anthony Central Hospital in Denver in the early 1980’s. He went on to earn a master’s degree in public health from the University of Colorado followed by his doctor of osteopathic medicine from the University of Health Sciences in Kansas City, Missouri. After returning to Colorado to complete his residency in family medicine at St. Anthony Central Hospital, Dr. Esfahani remained on staff as a physician. He received additional training in Geriatric and Rehabilitation Medicine and is the Medical Director of the Acute Rehab Unit at St. Anthony Hospital in Lakewood. He is also a member of the medical staff at Exempla Lutheran Medical Center in Wheat Ridge and has functioned in various leadership positions for several years there.
In addition to his above roles, Dr. Esfahani is the founder of Long Term Care & Rehab Consultants, LLC which provides medical direction, consultation, rehab medicine and primary medical care in acute, sub-acute and long-term care facilities. Dr. Esfahani is a recognized expert in transition of care— the movement patients make between health care practitioners and settings as their condition and care needs change. He and his clinical team direct patient care in several long term and skilled rehab facilities along the front-range. As a Certified Medical Director, he also lends his expertise to many area facilities and has been a major contributor in the development of an effective approach to transition patients smoothly from their PCP, hospital, nursing home and home.
Dr. Esfahani was born in Iran and came to the United States after finishing high school and serving two years of required military service during which he patrolled the borders of Pakistan, Afghanistan, Iraq and Kuwait. When not practicing medicine, Dr. Esfahani enjoys spending time with his wife and their three kids. He is also an avid tennis player and enjoys soccer and skiing.
Geoff was born in Golden, Colorado. His road to medicine started at the US Air Force Academy where he majored in Biology. He went on to serve in the United States Air Force where he was a Flight Commander Supervising Air Force, Army, Navy, Marine, and Coast Guard intelligence professionals. His time with the Air Force took him to Afghanistan where he was awarded a Bronze Star for his service. After leaving the Air Force, Geoff enrolled in the Physician Assistant program at the University of Colorado. In addition to his MPAS, Geoff has a Master’s in Public Health. He is most passionate about managing the multiple complex medical issues many of our patients endure. He works daily to help our patients and their families successfully navigate aging and end of life.
Geoff lives in Lakewood with his longtime girlfriend and two daughters. When he’s not seeing patients he enjoys skiing, climbing and running.
