Who will be managing my (or my loved one’s care) while in the facility?
One of our clinical providers, a physician, nurse practitioner, or physician assistant, will conduct a physical exam and review your case to determine an appropriate plan of care. From this evaluation, a care plan will be implanted and directed to the care team at the nursing home or rehab center.
How often will I see a doctor?
How often one of our clinical providers examines you will depend on your diagnosis and current condition. Some conditions warrant daily visits by one of our providers while other require only monthly visits.
Who should I contact if I have concerns about my or my loved one’s care?
If you have concerns about your care or the care of a loved one, your first contact should be with a member of the nursing home or rehab center staff. They can best determine who should further handle your concern. Typically, questions about prescription medications and health concerns will be addressed by our clinical staff, while issues with the comforts of your stay will be addressed by facility staff. We have clinical staff available to discuss medical concerns with your nurse 24 hours a day/seven days a week while in the nursing home or rehab center.
